Risk and Assumption of Risk Forms
At times it may be necessary to engage students in educational experiences outside the classroom and outside what is outlined in the syllabus.  It is possible that the experience may involve some risks not present in the classroom.  If it is determined that the experience could involve unusual risks, the Release and Assumption of Risk Form needs to be completed in full and a complete description of the risk of the activity should be documented.  If you have any questions concerning the need to use the form, please call.
